In response to Donnelly and Paton's article on the current state of knowledge on the safety of antidepressants during pregnancy, we would like to point out that earlier this year NICE published a guideline on antenatal and postnatal mental health, which deals with the issues raised. For example, the guideline recommends that where treatment with paroxetine is normally recommended (such as in the treatment of depression, GAD or OCD) a safer drug be considered instead. It also covers the safety of other psychotropic medication, including tricyclic antidepressants, valproate, lithium, and antipsychotics, and makes specific recommendations regarding discussing treatment plans with women who are planning a pregnancy, or already pregnant or breastfeeding.
